Coordinated care planning and continuum of care between the Acute Care Sector and Primary Care Sector
Chronic Disease Management (CDM) is working closely with the ACT Medicare Local (ACTML) and other relevant health care groups including the GP Liaison Unit, the Canberra Hospital and Winnunga Nimmityjah (ACT Aboriginal Medical Services) to develop a systematic approach to integrating CDM services with primary health care. The aim is to improve continuity of care for patients discharged from the acute care system into primary care, and vice versa.
Coordinated care planning and coninuum of care for Aboriginal and Torres Strait Islander patients of the CDMU
A particular component of our efforts to better integrate with primary health care is development of a strategy to extend CDMU services and other relevant services within the Division of Medicine to the Aboriginal and Torres Strait Islander populations in the ACT in an accessible and culturally appropriate manner. This Strategy works within the auspices of the ACT Chronic Disease Management Strategy relevant to Aboriginal and Torres Strait Islander issues and the memorandum of understanding with Winnunga Nimmityjah, the ACT Affiliate for Aboriginal Community Controlled Services in the ACT.
