Before you begin
Applications for the Allied Health Awards for Excellence are open to all eligible allied health professionals from ACT Health Directorate, Calvary Public Hospital Bruce, Canberra Health Services, Community Services Directorate, Education Directorate or Justice and Community Safety Directorate, who are either permanently employed or have been on a continuous temporary contract for at least 12 months at the closing date -10.00am Monday 9 August 2021.
Nominations can be made by Executives, Directors of Allied Health, Profession and Clinical Leads, managers, peers, consumers or self.
All nominations, including self-nominations, must be endorsed by the nominee’s current Executive. Consumers submitting a nomination should contact the Office of Professional Leadership and Education on Ph. 5124 9545 or AlliedHealthScholarShips@act.gov.au, for assistance in identifying the relevant Executive.
This form contains a number of mandatory fields. You will not be able to move through the form until information is entered in these fields.
You will be able to save a draft to access later however this draft will only be available on the workstation where you initially access the form, and the draft will only be available for 90 days.
Nominators will receive an email on successful submission of the nomination form. The identified Executive will also receive a copy of the nomination form, and a link to endorse the nomination.
There are four criteria, and each must be addressed:
The nomination should align with the capabilities and behaviours listed under each criterion, however you do not need to address them all. Please select the most relevant. A minimum of 2 must be selected.
Responses can be a maximum of 500 words per criterion. Strong nominations will make use of the entire word.
Responses may be made using dot points or written in paragraph style. Please ensure you state specifically how the capabilities and behaviours have been demonstrated.
Supporting documentation should not be attached to the application. If required, the Assessment Panel may request evidence from the nominator to further inform their evaluation.
Nominations that include two or more people must be submitted either in the Team Excellence category, the Research Excellence category, or the Excellence in Provision of Services to improve Aboriginal and Torres Strait Islander Health category.
Early Career Excellence
An Early Career Allied Health Professional is an eligible allied health professional who commenced as a new graduate with an ACT Government Directorate or Calvary Public Hospital Bruce on or after 1 January 2019. It is important to note that early career allied health professionals may come from a range of age groups.
Allied Health Assistant Excellence
Eligible Allied Health Assistants, Peer Recovery Workers and Dental Assistants can be nominated in this category.
