Palliative Care ACT

Palliative Care ACT advocates for and promotes best quality palliative care in the ACT. The team provide education within the community and their volunteer service supports people and their families with care, compassion, respect and dignity.

Palliative Care ACT train and support their volunteers to help people and their families in their own homes, in residential aged care facilities, at Clare Holland House (ACT Hospice) and in hospital.

Palliative Care ACT also provides overnight respite, day respite and carer support in its home away from home respite house – Leo’s Place.

Palliative Care ACT also have a directory of community resources that may be useful for people who have a life-limiting illness and their families.  This is called D.A.I.S.Y. and can be accessed on the Palliative Care ACT website.

Community Options Palliative Care Support Program

Practical and Domestic Support

People who choose to have their end of life care at home require more assistance with daily care and activities as their ability to do things decreases. Family members and friends can assist with providing practical care such as showering and helping a person go to the toilet. Many times however there is a need to get additional assistance to help care for a person. 

Funding to help with the additional practical care that is required is available through My Aged Care, or people may already be accessing the National Disability Insurance Scheme (NDIS). Both these services have eligibility criteria.

Where there is a need for additional support in navigating services or the need for case management services, support is available through the Community Options Palliative Care Program. The Community Options Program Palliative Care Coordinator can assist people, their carer’s and families in managing practical in-home supports in the end stages of life.

The Palliative Care Program provides case management, in-home support and community-based support. They offer this service to people at the end of their life who wish to die at home or stay at home for as long as possible. They do not provide medical/clinical help.

In-home supports include: 

  • personal care
  • domestic help
  • social support
  • respite and case management.
