Chronic Disease Management

Chronic Disease Management is a multidisciplinary team whose goal is to promote health, provide education and improve the self-management of people with chronic diseases in the ACT and surrounding regions, including heart failure, chronic obstructive pulmonary disease, obesity, Parkinson’s disease and other movement disorders.

Chronic Disease Management comprises the Chronic Disease Management Unit and Chronic Care Program in building 1 level 8A at The Canberra Hospital and the Obesity Management Service at the Belconnen Community Health Centre.

Living a Health Life with Long Term Conditions course

ACT Health runs this course for people with a chronic condition and their carers or friends.  A chronic health condition can be any condition lasting six months or longer such as asthma, arthritis, cancer, obesity, diabetes, chronic pain or heart disease.
