Rehabilitation, Aged and Community Services – Canberra Health Services

Rehabilitation, Aged and Community Services provides integrated and effective services for rehabilitation, aged care and community care throughout the ACT, including healthcare and support for people with acute, post-acute and long-term illnesses.

We aim to improve the quality and accessibility of services to clients and promote care that covers the prevention, assessment, diagnosis, treatment, support, rehabilitation and maintenance of conditions. We work closely with other healthcare providers to improve communication between primary, acute, subacute and community healthcare services.

Appointments for available services can be booked through any of the Community Health Centres, between 8am and 5pm, Monday to Friday and 8am to 3:30pm on Wednesday.
