Our Community Care Occupational Therapy service can help you to take care of yourself and be safe at home.

About this service

We provide occupational therapy services to help you become independent and safe in your home. Our therapists assess and make recommendations for equipment and home modifications including:

  • equipment, such as shower chairs, raised toilet seats or wheelchairs
  • home modifications, such as rails and safer ways to move around your home, and bathroom modifications.

Who we care for

Our service is available for ACT residents over the age of 18, and not living in a nursing home or currently an inpatient of a public or private hospital.

We offer these services for free if you have a Medicare Card or an Asylum Seeker Card.

There may be costs associated with the equipment and/or home modifications we prescribe.

How to access this service

You can make an appointment by calling Central Health Intake on (02) 5124 9977.

If you are 65 years or older (55 or older for Aboriginal, Torres Strait Islander people) you need to contact My Aged Care to be assessed for this service.

If you are on a level 3 or 4 home care package you need to contact your package provider for assistance.

If you are an NDIS participant there is a fee for service from your NDIS plan. We have more information about using your NDIS plan for our services.

If you are a Department of Veterans Affairs (DVA) gold card holder, you cannot use this service and need to contact DVA for a list of private providers.

What to expect

Our service prioritises appointments based on your needs.

We will contact you to ask some questions and arrange a visit to your home.

There may be a wait for an assessment, but we will send you a letter to let you know when we have received your details and then contact you by phone when an assessment time becomes available.

What to bring to your appointment

  • Any letters or correspondence from your GP
  • List of medicines you are currently taking (or the boxes), including medicines you have bought without a prescription, such as herbal supplements and vitamins
  • your NDIS plan or My Aged Care Plan (if you have one).
