Community Care Program

Community Nursing and Community Allied Health Services

The Community Care Program provides services in a community setting to ACT residents. The program is a registered provider for the National Disability Insurance Scheme (NDIS) and the Commonwealth Home Support Program (CHSP).

Medicare and Asylum Seeker cardholders not eligible under alternate funding can access these services free of charge.

About our services


If you have an impairment that is likely to become permanent and makes it difficult for you to participate in everyday activities, please contact the National Disability Insurance Scheme on 1800 800 110 or through their online contact form.

If you are over the age of 65 or an Aboriginal or Torres Strait Islander person over the age of 50, please visit the Department of Health or call My Aged Care on 1800 200 422 to access the Commonwealth Home Support Program. This program is available for Community Nursing, Podiatry, Physiotherapy and Occupational Therapy.

If you are a GP or health practitioner, please complete an online My Aged Care Health Practitioner Referral Form. Ensure you make a record of the Referral Number.

Our Community Care Program offers a collaborative approach to primary health care with specialised clinics for management of wounds, continence and stoma. Self-management of chronic conditions, adult healthy weight and chronic back pain management classes are also available.

Community Care - Nursing

We offer clinical nursing services including wound, catheter, stoma and drain management, palliative care, medications administration and education.

These services are usually delivered in Community Health Centres.  A limited home visiting service is available.

Our nursing services operate from Monday to Friday, with limited services available on the weekends with a booking.

The Link Team also provides after hours community nursing services, including palliative care.


Our community care nursing and allied health service offers assessment and management of continence and pelvic pain issues. We also provide pre-prostatectomy and pelvic floor education sessions.


We offer assessment and ongoing general podiatry care to those who are at risk of serious foot problems including the treatment and prevention of wounds. We also offer short term biomechanical assessment where our Podiatrists can prescribe and supply foot orthoses and in-grown nail management including nail surgery. We provide physiotherapy services for a wide range of conditions, including muscle and joint strains and sprains, arthritic conditions, neck and back conditions and recovery after surgery.

We also offer Pelvic Health Physiotherapy to assess and manage a range of continence and pelvic pain issues for both men and women. For more information visit Physiotherapy Service.

Occupational Therapy

We provide occupational therapy services to help you become independent and safe in your home. Our therapists assess and make recommendations for equipment and home modifications.

We offer dietary assessments, advice and counselling for nutrition related conditions, including minor malnutrition, chronic diseases, kidney disease, gastro-intestinal conditions and wound healing.

We also run a nutrition service for people who are tube fed at home, as well as an Adult Healthy Weight introduction group for overweight people requiring the latest healthy eating advice. For more information visit Community Care Nutrition Service.

Social Work

We offer short-term counselling and emotional support for social and psychological issues such as adjustment to changes in health conditions, anxiety and depression, loss and grief counselling, advocacy, information and referral pathways.

This service is also available to partners, family members, carers and meaningful others who are affected by illness.
