Rehabilitation, Aged and Community Services provides integrated and effective services for rehabilitation, aged care and community care throughout the ACT, including healthcare and support for people with acute, post-acute and long-term illnesses.

We aim to improve the quality and accessibility of services to clients and promote care that covers the prevention, assessment, diagnosis, treatment, support, rehabilitation and maintenance of conditions. We work closely with other healthcare providers to improve communication between primary, acute, subacute and community healthcare services.

Important changes to NDIS in-kind funding arrangements

Rehabilitation, aged and community care services

From 1 July 2019, we are changing from being an ‘in-kind’ to ‘fee for service provider’ for the National Disability Insurance Scheme (NDIS). Previously our services were pre-paid under the NDIS. You will still be able to access the same supports you need from us but you will need to have funds allocated in your NDIS plan to pay for these supports. 

For current clients of the affected services, we will work with you and the National Disability Insurance Agency (NDIA) to ensure this change occurs smoothly and you continue to receive the supports you need throughout the transition process.

How you access our services will now be the same as for other NDIS service providers in the community.

How does this change affect me?

You will still be able to access the same services from us if you wish. We will work with you to check that you have funds in your plan for the NDIS supports we are currently providing you.

If you do not have supports you need from us listed on your plan, we can assist you by providing advice to the NDIS regarding the supports you have been accessing, or require.  Please be assured that you won’t be left without supports you are currently receiving during this transition.

What do I need to do?

If you would like to access NDIS funded services from us after 1 July 2019, please check that the supports we provide you are listed and properly covered through your NDIS plan. If they are not we encourage you to talk to your planner, service coordinator, or the local area coordinator Feros Care.

If you are currently receiving services and would like to continue to do so, before 1 July or at your next appointment you should:

  • check that the supports our services provide are listed and properly covered on your NDIS plan
  • provide us with a copy of your plan, or the relevant parts, if you haven’t already
  • complete a Service Agreement with us for each service you receive from us.

If you aren’t currently accessing supports through us but need to in the future, you can call Central Health Intake on 02 5124 9977 to book. You will be asked if you have funding for the requested supports on your NDIS plan. You will be assisted to ensure you can access necessary supports.
